I completed the American Association of Colleges of Nursing (AACN) (2008) self-assessment where I scored 35. In my nursing career, I have used both paper and electronic charting methods in documenting patient health data. Charting in the different facilities I have worked in was always different but I have always managed to maneuver through them.
