Ms. George is a 32-year-old computer programmer. Over the last several months, she has had increased episodes of a burning sensation in the mid epigastrium and back. The pain subsides after eating. Based on her history, the physician orders an endoscopy that reveals several peptic ulcers. Treatment of the ulcers includes antibiotics, proton pump inhibitors, and bismuth salts. Solution

  • Peptic Ulcer Disease – is a disruption in the continuity of the lower esophageal gastric, or duodenal mucosa that leads to a local defect resulting from inflammation. It may develop in any part of the gastrointestinal tract that has a constant relationship with gastric juices or the hydrochloric acid and pepsin in our stomach. It may be arising from the esophagus, stomach, duodenum or jejunum. This may be characterized by open sores that develop on the inside lining of your stomach or other parts of the GI tract. It may be characterized by a burning sensation that usually occurs after meals. It can lead to internal bleeding which can result into blood transfusions.
    • Duodenal ulcer – A peptic ulcer that develops in the duodenum.
    • Gastric ulcer – Mostly found in the distal to the junction between the antrum and the acid secretory mucosa.
  • Etiology: 90% of peptic ulcers are caused by pylori which is the only classified Class I carcinogen by the World Health Organization.

Apart from the presence of the said bacteria, the volume of hydrochloric and biliary acids also contributes in developing peptic ulcers. At the same time the following risk factors also contribute to the development of this disease:

  • Smoking
  • Steroids
  • Aspirin
  • NSAIDs
  • Caffeine
